Our patient portal, My Patient Visit, allows you to submit your registration and health history information online in the comfort of your own home.  Your information then merges directly into your electronic medical record.  This way, you don't feel rushed completing forms in the office prior to your appointment. You can also access a clinical summary of each of your visits via the patient portal.
